  • Overview

    The Care Coordinator will work with the Emergency Department team to ensure that all patients discharged from the emergency room receive timely, appropriate post-discharge follow up care. The Care Coordinator will assist patients with appointment scheduling, insurance approval, care team management, and health record coordination.  The Care Coordinator will also be working closely with the sponsoring physician(s) for program improvement and marketing.


    • Assist the patients in finding a PCP if a previous relationship has not been established.
    • Schedule appointments with the patient’s PCP.
    • Schedule appointments with specialists and coordinate the physician referrals.  The Care Coordinator will also follow up with the patients to confirm attendance of the specialist appointments.
    • Compliance with hospital and ProScribe policies, including those relating to HIPAA and Joint Commission
    • Regular and reliable attendance at assigned facility required
    • Other duties as delineated by the sponsoring physician(s) within the Care Coordinator’s scope of practice.

    Education/Experience: High school diploma required. College sophomore status or higher preferred. Pre-health career track preferred. Ability to demonstrate knowledge of medical terminology and human anatomy strongly preferred.


    Technical/Computer Skills: Skilled in operating various medical record software and hardware, word-processing, and database software programs; minimum of 60 WPM preferred.

    • Working knowledge of medical terminology and abbreviations
    • Understanding of the patient discharge process and ability to schedule follow up appointments
    • Legible handwriting and ability to record information accurately
    • Computer proficiency and ability to learn new applications quickly including the facility’s EMR and Care Continuity Program
    • Communication skills and the ability to coordinate and cooperate with multiple members of the healthcare team
    • Professional demeanor and recognition of privacy considerations with respect to patients and families
    • Strong written and verbal communication skills.

    Performance Assessment:


    Performance will be assessed in a written format during a 90-day probation period.
